How to enroll a loved one

Whether you’re caring for a family member or a friend, Prescription Hope makes it easy to enroll a loved one. 

Creating an account

If you’d like to create an account for an individual other than yourself, fill out the enrollment form with the patient’s information and not your own.

All we will need from you is your name and phone number under the enrollment form’s alternate contact field.

All other fields will require the patient’s information.

On step two of the online enrollment form, there is a field for an “Alternate Contact.” In this field, place your information.

Adding yourself as an alternate contact will allow you to call in and ask questions about the patient’s account on their behalf.
